Your first visit with Colorado Health and Youth Dermatology Centers establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You can even save time at your first visit by printing and completing the patient forms in advance of your appointment.

First time patients, let us know it's your first visit. E-mail or fax (813)661-0031 you insurance informationNew Patients. We will call you back to schedule an appointment convenient for you. Once you have your appointment, plan to arrive ten minutes early in order to provide or complete additional patient information registration paperwork. Please bring with you the following:
- Your insurance cards
- Photo ID, Drivers License
- S.S.N. of person responsible for the patient account
- Referral forms (if required)
- List of all prescriptions and Over the counter medicines
- Any required co-payment is due at time of check-in
- Pertinent medical or surgical history information

At Colorado Health and Youth Dermatology Centers our model of medical care includes a broad holistic approach to medicine which includes Cosmetic Medicine as well as Wellness Medicine and Nutrition. We believe in a strongly personalized approach to the practice of medicine with the center being the patient-Physician relationship to arrive at the best understanding of the patients need. Our goal of understanding the whole person allows us, we believe, to provide the best care for the patient. - Brandon Office: 507 Oakfield Drive, Brandon Fl. 33511. The office is on the same street Brandon Hospital is on. About one block west of the hospital on the same side. Across the street from Music Showcase, on the corner of Eichenfield and Oakfield drive.
- North Tampa Office: 13701 Bruce B. Downs Blvd. suite # 108 Tampa Fl 33613. The office is in a single story, red-tiled roof medical complex of offices. It is one block north of Fletcher avenue and on the east side, next to University Community Hospital. Across from Wendy's.
- Davis Island medical Spa-La Vida Bella Medical Spa: 233 East Davis Island BLVD. 233 East Davis Blvd., Tampa, Fl. 33606. The office is on the East side of the street. Across from 220 East Restaurant.
